THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED, AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Our commitment to your privacy
Frontline Primary Care is required by law to maintain the privacy of protected health information (PHI), to provide you with this Notice of our legal duties and privacy practices regarding PHI, and to abide by the terms of the Notice currently in effect.
How we may use and disclose your PHI
For treatment
We may use your PHI to provide medical care, including coordinating with other providers and services involved in your care.
For payment
We may use and disclose your PHI to obtain payment for the care we provide — for example, to verify insurance coverage, submit claims, and respond to questions from your health plan.
For health care operations
We may use your PHI for activities necessary to run our practice and ensure quality care — including quality assessment, training, audit and compliance, and business management.
Other permitted or required uses and disclosures
- When required by law (e.g., reporting certain communicable diseases or suspected abuse).
- For public health activities and health oversight.
- For judicial and administrative proceedings, when properly authorized.
- For law enforcement purposes, as permitted by law.
- For certain research purposes that have been approved by an institutional review board.
- To avert a serious threat to health or safety.
- For specialized government functions (military, veterans, national security, etc.).
- For workers' compensation, as permitted by law.
Uses and disclosures requiring your written authorization
Other uses or disclosures of your PHI not described above require your written authorization. You may revoke that authorization at any time, in writing, except where we have already acted in reliance on it. The following always require your written authorization:
- Most uses and disclosures of psychotherapy notes (when applicable).
- Uses and disclosures for marketing purposes.
- Sale of PHI.
Your rights regarding your PHI
- Right to inspect and copy your PHI in our designated record set.
- Right to request amendment of PHI you believe is incorrect or incomplete.
- Right to an accounting of certain disclosures we have made of your PHI.
- Right to request restrictions on certain uses and disclosures (we are not required to agree, except in the limited case of disclosure to your health plan for services you paid for in full out of pocket).
- Right to request confidential communications by alternative means or at alternative locations.
- Right to a paper copy of this Notice, even if you have agreed to receive it electronically.
- Right to be notified of a breach of unsecured PHI affecting you.
Changes to this Notice
We reserve the right to change this Notice and to make the changed Notice effective for PHI we already have, as well as PHI we receive in the future. We will post the current Notice in our office and on this website.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.
