Welcome. Here's what to bring.

A few small things before your first visit make everything go smoother. We'll call ahead to confirm — but the list below is a great starting point.

What to bring

The first-visit checklist.

Don't stress if you forget something — we can usually work around it. But these speed things up.

  • Photo ID

    Driver's license, state ID, or passport.

  • Insurance card

    Front and back. We'll make a copy.

  • Medication list

    Names, doses, and how often you take them — including OTC and supplements.

  • Recent labs or imaging

    If you have results from another doctor in the last 1–2 years, bring or forward them.

  • Conditions & allergies list

    Chronic conditions, surgeries, drug or food allergies, family history if you know it.

  • Emergency contact

    Name and phone number of someone we can reach in an emergency.

Insurance & payment

Most major plans accepted.

We accept most major insurance plans for primary care visits. Coverage varies by carrier — call (918) 455-2416 and we'll confirm your specific plan in a minute. Bring your insurance card to your first visit.

Cash-pay welcome. If you don't have insurance, or your plan isn't accepted, ask us about self-pay rates.

Medical weight loss is cash-pay only. Most plans don't cover compounded GLP-1 medications. The advantage of cash-pay is predictable monthly cost and no prior-authorization delays.

What to expect at your first visit

    Arrive 10 min early

    Quick check-in, ID and insurance verification. We try to keep paperwork minimal.

    Vitals & history

    Blood pressure, weight, brief health questions before you see Dr. Kim.

  3. 03

    Time with Dr. Kim

    30–45 minutes. Real conversation. Exam if appropriate. No 7-minute slots.

  4. 04

    Plan & next steps

    We outline labs, medications, follow-ups. You leave knowing what's happening and when.
